Summer Review Appointment Request

Summer Review – Reading and Math for Grades 1-8

Request an appointment for three, one-on-one sessions with a licensed teacher to review reading or math skills over the summer. A parent or guardian must be present to register the student at the first session.

Library staff will contact you to schedule appointments based on teacher availability.

  • By submitting this form, you agree that you have read and understand the following:
    • Cancellations must be made at least 24 hours in advance. Call Shaker Main Library at 216-991-2030, option 7, to cancel an appointment.
    • A cancellation less than 24 hours in advance will be considered a “no show.”
    • More than one “no call, no show” will result in the cancellation of future appointments.
